Our Founding NP’s Washington licensure is in process. As soon as the state issues the license, we begin onboarding Washington patients. Join the Blueprint list and we will notify you the day care opens.
You need to be a Washington resident receiving care while in Washington. We confirm your address during intake. Telehealth law requires the provider to be licensed in the state where the patient is physically located at the time of the visit.
Membership is $295/month nationwide and includes comprehensive labs at month one and every 3 months. Medications are billed separately by your pharmacy and can be processed through your insurance like any other prescription.
The Elysiv membership fee is cash-pay; HSA/FSA accepted. Labs are included in the membership. Prescriptions can be filled at any pharmacy and processed through your insurance like any other prescription.
