Elysiv Life Management Services LLC is required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this notice, and follow the terms of the notice currently in effect.

We may use and disclose your PHI for treatment, payment, and health care operations, as well as for purposes permitted or required by law (such as public health reporting or court orders).

Your rights

You have the right to inspect and copy your record, request an amendment, receive an accounting of disclosures, request confidential communications, request restrictions on use and disclosure, and receive a paper copy of this notice.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with Elysiv (hello@elysivlife.com) or with the Secretary of the U.S. Department of Health and Human Services. There will be no retaliation.
