What telehealth is
Telehealth involves the delivery of health care services through real-time video, audio, or secure messaging. Your provider at Elysiv may not be physically present with you during care.
Benefits
Telehealth improves access to care, reduces travel, and can speed diagnosis and treatment. You may receive the same quality of care as an in-person visit for many conditions.
Limitations
Some conditions require in-person evaluation. Your provider will refer you to in-person care when clinically appropriate. Technical issues may occasionally disrupt a visit.
Privacy
Telehealth visits occur over HIPAA-eligible platforms. Our providers will not record sessions without your express written consent.
Emergencies
Telehealth is not appropriate for emergencies. If you experience a medical emergency, call 911 or proceed to the nearest emergency room.
Consent
By booking a visit with Elysiv, you acknowledge that you have read this notice and consent to receive care via telehealth. You may withdraw consent at any time; withdrawal will end future telehealth visits.
