You need to be an Arizona resident receiving care while in Arizona. Telehealth law requires the provider to be licensed in the state where the patient is physically located at the time of the visit.
Membership is $295/month nationwide and includes comprehensive labs at month one and every 3 months. Medications are billed separately by your pharmacy and can be processed through your insurance like any other prescription.
The Elysiv membership fee is cash-pay; HSA/FSA accepted. Labs are included in the membership. Prescriptions can be filled at any pharmacy and processed through your insurance like any other prescription.
