You need to be an Arizona resident receiving care while in Arizona. Telehealth law requires the provider to be licensed in the state where the patient is physically located at the time of the visit.
Membership is $395/month and includes comprehensive labs at baseline and every 3 months, a body-composition smart scale, monthly video follow-ups with your provider, a written protein and strength plan, a supplement plan and off-ramp planning. Medication is not included; it is prescribed only when appropriate and paid directly to the pharmacy or manufacturer program, and can be processed through your insurance like any other prescription.
Elysiv does not bill insurance. The membership fee is cash-pay; HSA/FSA cards are accepted, with eligibility determined by your plan. Labs are included in the membership and ordered under Elysiv’s account, so nothing is billed to your insurance. Prescriptions can be filled at any pharmacy and processed through your insurance like any other prescription.
