1. Acceptance

By accessing or using elysivlife.com and the services offered by Elysiv Life Management Services LLC (“Elysiv”), you agree to these Terms of Service.

2. Nature of Services

Elysiv is a licensed telehealth practice providing weight-management care in states where the provider is licensed. Use of the website does not by itself establish a provider-patient relationship. A relationship is established only after a clinician reviews your intake and meets with you.

3. Eligibility

Care is limited to adults 18 years and older residing in states where Elysiv is licensed. We currently serve Washington and Arizona; additional states are pursued as licensure completes.

4. Payment

All memberships are billed monthly. All sales are final. Cancellation ends future billing but does not refund past charges.

5. Limitation of Liability

Elysiv provides care within the scope of the provider's license. Elysiv is not liable for outcomes outside that scope or for third-party services (pharmacies, laboratories, payment processors).
