What we collect

When you interact with elysivlife.com, we may collect: name, email, state of residence, information you volunteer in the chatbot, and standard website analytics (pages visited, referrer, device type).

Intake privacy

Health answers in the intake form (conditions, medications, height, weight, prior GLP-1 use) remain in your browser and are not transmitted to our servers. Only your name, email, and state are sent to our email marketing system when you submit intake.

Clinical privacy

Once you become a patient, your Protected Health Information (PHI) is handled under HIPAA-compliant systems with appropriate business associate agreements. The marketing website does not store PHI.

Third-party processors

We use Mailchimp for email marketing, Resend for transactional email, Stripe for any payments, and Anthropic for the website chatbot. These providers each have their own privacy terms.

Your rights

You may request deletion of your marketing record by emailing hello@elysivlife.com. Clinical records are retained according to state and federal law.
